Stroke kills roughly 162,000 Americans each year and is the fourth leading cause of death among women in the United States. For women over 55, the risk climbs steeply as estrogen levels decline during and after menopause, stripping away a hormone that normally protects blood vessels, reduces inflammation, and supports healthy blood flow to the brain. The numbers are bleak, but a new study tracking more than 105,000 women over two decades offers concrete evidence that what you put on your plate can meaningfully shift the odds in your favor.
Researchers from the City of Hope Comprehensive Cancer Center, Charles R. Drew University, and Columbia University published their findings in Neurology Open Access on February 4, 2026, reporting that women who closely followed the Mediterranean diet were 18% less likely to have any type of stroke. The protection was even stronger for hemorrhagic stroke, the deadlier subtype caused by bleeding in the brain, where high-adherence women saw a 25% risk reduction. The study drew on the California Teachers Study, one of the largest and longest-running prospective cohort studies of women in the country, and its 21-year follow-up period makes it one of the most durable investigations of diet and stroke risk ever conducted.

The California Teachers Study began in 1995 when researchers enrolled 133,477 female educators and school administrators who were members of the California State Teachers’ Retirement System. For this stroke analysis, the team narrowed the cohort to 105,614 women with no prior history of stroke, all with an average age of 53 at baseline. Wang, PhD, molecular epidemiologist at the City of Hope Comprehensive Cancer Center and lead author of the study.
